What happened to Captain MacMillan?
Modern Warfare After Pripyat, MacMillan left field duty and retired, but was later promoted to Major-General presumably around 1996 to 2016, and Price was promoted to Captain. Captain Price and his team later succeeded in killing Zakhaev, but the Ultranationalist leader became a war hero of Russia.
How did Soap survive?
Soap pushes Yuri out the hole in the wall to save him, but gets caught in the blast radius, accompanied by suffering a long fall. Soap survives the fall, but is severely injured as a result of the blast, the fall and debris collapsing on him.
